MATHEW WILL OCCUR ON FRIDAY MAY 28 AT 8 A.M.  Your student must have the “Medical Questionnaire” Part A completed by the parent/guardian, and the letter “C.” signed by the parent/guardian on the reverse side of the form.  Students who do not have a completed and signed physical form (Medical Questionnaire) will not be able to get a physical.
